Connect to Care is part of a national initiative put in place by the Administration on Aging (AoA) and the Centers for Medicaid and Medicare Services.

Connect to Care is a web-based resource center created to provide access and assistance to older adults and individuals with disabilities needing information on the resources available. This web-based resource center may also serve as a source for those planning for future long-term care needs. Connect to Care strives to serve as the entry point for professionals, caregivers, and family members who are seeking resources and assistance on behalf of their clients or family members.
